The Center for Excellence in Primary Care (CEPC) exists to develop, identify, and spread promising innovations in primary care, particularly in the safety net that cares for vulnerable populations. CEPC provides training in team-based care, offers practice facilitation to spur and support transformation, broadens the evidence base through research and evaluation, and advocates for policy that promotes high performing primary care.

The UCSF Center for Vulnerable Populations (CVP) at Zuckerberg San Francisco General Hospital and Trauma Center is dedicated to improving health and reducing disparities through discovery, innovation, policy, advocacy, and community partnerships. The CVP seeks to develop effective strategies to prevent and treat chronic diseases in communities most at risk.

The International Centre for Health Innovation at the University of Western Ontario hosts Innovatexchange, an interactive and collaborative map of innovations across the health care continuum.

The Department of Health Services (DHS) provides acute and rehabilitative patient care, trains physicians and other health care clinicians, and conducts patient care-related research. DHS operates four hospitals, including some of the nation's premier academic medical centers through their affiliations with the University of Southern California School of Medicine, UCLA School of Medicine and the Charles R. Drew University of Medicine and Science. In addition, DHS operates six comprehensive health centers and multiple health centers throughout Los Angeles County, many in partnership with private, community-based providers.

The San Francisco Community Clinic Consortium (SFCCC), a partnership of eleven community clinics and health centers (eight FQHCs, two community clinics, and one free clinic), collectively serves more than 94,000 patients annually. SFCCC is the federal Health Care for the Homeless grantee, Ryan White Part C Grantee, and Area Health Education Center (AHEC) for San Francisco. SFCCC is an integral part of the health care system of San Francisco.

Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) is a licensed general acute care hospital, which is owned and operated by the City and County of San Francisco, Department of Public Health. ZSFG aims to provide quality health care and trauma services with compassion and respect.

San Francisco Health Plan (SFHP) is a licensed community health plan that provides affordable health care coverage to more than 55,000 low and moderate-income San Franciscans. SFHP is a City-sponsored (“local initiative”) plan which provides its members with high-quality medical care and supports San Francisco’s public and community health organizations.

UCSF and San Francisco General Hospital have been partners in health care since 1873. The University provides faculty from all four schools – dentistry, medicine, nursing, and pharmacy – who provide patient care services, conduct research, and teach at ZSFG.
